Services at Phoebe Sumter Hospice
While known for emotional support, Phoebe Sumter Hospice also offers the best in symptom control and pain management. For those with a life-limiting illness, the hospice team works with patients to identify specific needs and develops a plan that addresses those needs. This care is provided in collaboration with the patient’s physician and the hospice medical director.
As a hospice patient, you can expect regular visits from a caring team of professionals who work with the doctors that you may already have. You can stay right at home, surrounded by the people and the things that you love. Our nurses, home health aides, social workers, chaplains and community volunteers will come right to you.
The same excellent service that we provide at home is also available for residents of local nursing homes, assisted living facilities and personal care (or senior living) homes.
Routine Hospice Care Services Include:
- Care directed by a doctor who has training in providing hospice and end-of-life care. Your loved one’s primary care physician remains in control, with assistance from the hospice care team.
- Regular visits to the home by nurses who have training in hospice care, as well as 24-hour, on-call nursing support
- Help with daily personal care needs
- Visits from the hospice chaplain for spiritual support and encouragement
- Counseling and social services provided by trained social workers
- Medical supplies and equipment such as: hospital beds, bedside commodes, wheelchairs and oxygen
- Medications to help control pain and other symptoms
- Bereavement care for the family following the patients’ or loved ones’ death
- Access to therapies, such as physical, occupational, speech and dietary therapies, as needed, depending on your loved one’s condition. Other complimentary therapies may include: art therapy, play therapy, music therapy, pet therapy, massage therapy, horticulture therapy and essential oil therapy.
