What is Phoebe Patient Portal and who can use it?

The Phoebe Patient Portal is a convenient and secure online health information management tool patients can use to view protected healthcare information wherever internet access is available. Information that is currently available includes:

  • Pay office bills online (not hospital bills)
  • View medical records
  • View radiology and lab report
  • View physician reports
  • Emergency room visit records

Patients who have used any hospital within Phoebe Putney Health System (PPHS) and are 18 years of age or older. Parents can complete a Child Proxy request and have access to their minor children’s information. Patients can also give another adult access by completing the Adult Proxy Request Form and guardians/healthcare agents who have a Durable Power of Attorney can request access by completing the Adult Proxy DPOA.

How do I get started and how can I access Phoebe Patient once I have completed the account setup process?

On the log-in page, click on the “Enrollment Form” link in the left lower corner. After the account setup process is complete, patients can log in by clicking the “Hospital Visit Portal” button at PhoebePatient.com

Who do I contact if I have trouble logging in or accessing Phoebe Patient, and what should I do if I forget my password?

If you experience trouble logging in or accessing Phoebe Patient, please call 229-312-5465, Monday – Friday from 8 a.m. – 5 p.m. If you register for Phoebe Patient and forget your password, you can simply click the “Forgot Password?” link below the sign-in box on the sign-in page.

Can I obtain medication refills or pay my hospital bill in this portal?

Phoebe Hepatology, Phoebe Sumter Ellaville, Phoebe Rheumatology, and Phoebe Worth Family Medicine patients can request medication refills through the Patient Portal. Other medication refills cannot be made through the portal, at this time. You will need to contact your physician’s office for refills. Likewise, hospital bills cannot be paid through this portal. Can I access the portal on all my electronic devices, and will I be notified if my session is going to time out?

The Patient Portal is available through the Phoebe Access app that is located in the Apple App Store or Google Play Store. Yes, there is also a mobile app for the patient portal. MEDITECH mHealth can be found in the Apple App Store or Google Play Store.

Yes, there is a time-out notification that will notify you after 15 minutes of inactivity. The notice will give you 1 minute to save your data. Note that there is no “save” feature with the time out notification.

How far back can I review my health record on Phoebe Patient?

Fall 2015 to present.

Will Phoebe Patient show my recent visit to my Phoebe Physician office or clinic?

Phoebe Patient does show recent visits to Phoebe Primary Care offices. To see records from your Phoebe Physicians doctor’s office visit, you must click ‘Office Visit Portal’ above or simply click here to log-in/sign-up. You may also contact the Phoebe Physicians customer service line at 229-312-5802.

Patients at Phoebe Worth Family Medicine, Phoebe Sumter Ellaville, Phoebe Hepatology, and Phoebe Rheumatology can access medical records through ‘Hospital & Selected Office Locations Visits’ link.

Where are the records from my Urgent Care visit?

Urgent Care visits can be found at Phoebe Patient as an ‘Office Visit Portal‘. Patients can contact customer service line at 229-312-5802 if he/she has any questions.

Who can change my health record?

As the patient, you can update your profile information and submit for approval by Phoebe.

If you feel information in your record is incorrect, please contact Health Information Management (HIM) Department at 229-312-6000 to complete an Amendment Request Form. HIM cannot update your medical record, but will work with your providers to update the medical record..

How can I obtain a copy of my medical record?

Phoebe Patient is the quickest and easiest way for you to access your medical records from your computer or cell phone. You can also email [email protected] to request your medical records. Please include a call back telephone number, patient’s full name, date of birth, the visit date of the records needed, and the address where the medical records can be mailed or if pick-up is preferred. All requests will be processed within 3 business days or patient will be notified of any delay.

Additionally, patients may come to the Health Information Department to request records in person. Please remember to bring a copy of a picture ID. While we strive to provide records at the time of request that is not always possible.
