Policy Guidelines

Effective Monday, February 14, 2022 all vendor representatives who access any Phoebe facility must have valid proof of receiving the J&J COVID vaccine or start the two-dose vaccine series with the Pfizer or Moderna vaccine or have an approved Phoebe medical or religious exemption. The COVID-19 Vaccination Policy for vendors can be found here, GHX Vendormate Digital Badging remote access or at GHX Vendormate kiosks located at Phoebe Putney Memorial Hospital, Phoebe North Campus (will be installed on February 14, 2022), Phoebe Sumter Medical Center, and Phoebe Worth Medical Center. Vendor representatives must provide valid proof of full COVID-19 vaccination by March 15. Entering any Phoebe facility obligates the vendor to abide by Phoebe’s COVID-19 Vaccination Policy. A copy of a vaccine card or an approved Phoebe exemption must be uploaded into GHX Vendormate by the vendor representative by February 14.

If full vaccination is not achieved, or an approved exemption is not provided, vendor representatives are not allowed into any Phoebe location.

All visiting vendor representatives must sign in through GHX Vendormate, upload required vaccination documentation or Phoebe approved exemption, attest they have reviewed this policy and print their badge upon arrival.

For any Phoebe location that does not have a GHX Vendormate kiosk, vendors are to utilize GHX Vendormate Digital Badging remote access capabilities or may be required to show proof of vaccination to their Phoebe contact prior to the visit or service.

Vendor’s exempt from GHX Vendormate registration requirements or do not have access to GHX Vendormate are required to review the onsite and visiting vendor vaccination guidance and sign an acknowledgement of Phoebe’s policy and standardized work instruction. Due to the nature of some services, our vaccination requirements will not extend to the following types of vendors:

  • Delivery, freight, shipping couriers, and logistics personnel
  • Utility service providers such as gas, electric, water, cable/internet and phone
  • Repair personnel needed to respond to any emergent issues to ensure continued operations and the protection of Phoebe property, patients and staff.
  • All local, state and federal government and regulatory agencies who would need to access facilities for official business.
  • Any service providers privately contracted and paid by any patient in our post-acute facilities.

Contractors and subcontractors with Phoebe badges follow same guidelines as employees. Contractors and subcontractors without badges follow GHX Vendormate process.

Please do not enter any Phoebe facility if you have symptoms including: 

  • Fever
  • Cough
  • Difficulty breathing
  • Body aches
  • Chills
  • A new loss of sense of smell or taste

Our patients have weakened immune systems and we must take every precaution to protect them from infection, including COVID-19.

All individuals are required to comply with Phoebe’s masking, screening and social distancing guidelines while on Phoebe campuses. Currently all individuals must continue to wear a mask in all patient care facilities, clinical areas, and public areas where patients may be present, regardless of vaccination status.

Failure to properly prepare for the February 14, 2022, effective date, will result in your inability to access Phoebe facilities. It’s imperative for your company to begin gathering documentation and uploading and attesting to this policy as soon as possible to avoid disruption.
