To qualify for home health services:
- Must have a need for intermittent skilled nursing or therapy services.
 
- The patient is willing and able to participate in their care and management.
 
- Must have a willing and able caregiver if the patient is not able to participate themselves without assistance of another person.
 
- Remains under the care of a physician who has authorized home health services.
 
- Must be homebound – taxing effort to leave home, unsafe to leave the home and leaves infrequently, etc.
 
- Has a new or exacerbated disease process.
 
- Must have a need for intermittent skilled nursing or therapy services.
 
 
What services are not offered or covered by Phoebe Home Health?
- Lab Draws only
 
- Venipuncture
 
- IV medication administration only due to billing groupings in Georgia (commercial insurance)- must have another service to go with it
 
- Personal care assistance only
 
- 24/7 care or sitting services
 
- Cooking/Cleaning services class/Household chores
 
- Medication set-up only
 
- Medical equipment needs
 
- Transportation services
 
- Medical social services
 
- Simple wound care beyond 3 weeks for teaching and monitoring
- Examples: Wet-to-dry dressings or just Neosporin and dry dressings /Band aids
 
 
- Psychiatric nursing
